2) GENERAL EXPLANATIONS
The rights of the data owner/relevant persons in accordance with Article 11 of the Law on the Protection of Personal Data No. 6698 (“Law”) are as follows:
- The right to learn whether your personal data is processed,
- If your personal data has been processed, the right to request information about it,
- The right to learn the purpose of processing your personal data and whether they are used in accordance with the purpose,
- The right to learn about the third parties to whom your personal data is transferred at home or abroad,
- The right to request correction of your personal data if it is incomplete or incorrectly processed and to request the notification of the transaction made within this scope to the third parties to whom the personal data has been transferred,
- Despite the fact that it has been processed in accordance with the provisions of the law and other relevant laws, the right to request the deletion, destruction or anonymization of your personal data in the event that the reasons requiring it to be processed disappear, and to request the notification of the transaction made within this scope to the third parties to whom the personal data has been transferred,
- The right to object to the emergence of a result against you by analyzing your processed data exclusively through automated systems,
- The right to demand the compensation of the damage in case you suffer damage due to the unlawful processing of your personal data.
As Data Owners/Relevant Persons, you can submit your requests within the scope of your rights mentioned above to ÖZPOR with your application based on Article 13 of the Law.
3) ISSUES NOT COVERED BY THE APPLICATION
Pursuant to Article 28 of the Law, since the following situations are not within the scope of the Law, it is not possible for personal data owners/related persons to assert their rights in accordance with Article 11 of the Law in the following matters:
- Processing of personal data for art, history, literature or scientific purposes or within the scope of freedom of expression, provided that it does not violate national defense, national security, public security, public order, economic security, privacy of private life or personal rights or does not constitute a crime.
- Processing personal data for purposes such as research, planning and statistics by making it anonymous with official statistics.
- Processing of personal data within the scope of preventive, protective and intelligence activities carried out by public institutions and organizations authorized by law to ensure national defense, national security, public safety, public order or economic security.
- Processing of personal data by judicial authorities or execution authorities in relation to investigation, prosecution, trial or execution proceedings.
Pursuant to Article 28/2 of the Law; It is not possible for personal data owners to assert their rights, with the exception of demanding compensation in the following cases:
- The processing of personal data is necessary for the prevention of crime or for criminal investigation.
- Processing of personal data made public by the personal data owner.
- Personal data processing is necessary for the execution of supervisory or regulation duties and for disciplinary investigation or prosecution by authorized and authorized public institutions and organizations and professional organizations in the nature of public institution, based on the authority given by the law.
- The processing of personal data is necessary for the protection of the economic and financial interests of the State with regard to budgetary, tax and financial matters.
4) APPLICATION METHOD
You can submit your requests within the scope of your rights listed in Article 11 of the Law, together with this form, to ÖZPOR using one of the 3 methods described below.

5) CONCLUSION OF APPLICATIONS
Your applications will be concluded free of charge as soon as possible and within thirty (30) days at the latest, depending on the nature of your request; however, if the transaction requires an additional cost, you may be charged a fee according to the tariff to be determined by the Personal Data Protection Board.
During the evaluation of the applications, ÖZPOR first determines whether the person making the request is the real right holder. However, when it deems necessary, ÖZPOR may request detailed and additional information in order to better understand the demand.
Responses to data owner applications are notified by ÖZPOR in writing or electronically. If the application is rejected, the reasons for the rejection will be explained to the data owner with justification.
NOTE: According to the nature of this form and your request, the information and documents requested from you must be provided to us completely and accurately. In case the requested information and documents are not provided properly, there may be problems in the full and qualified execution of the researches to be carried out by ÖZPOR based on your request. In this case, ÖZPOR declares that it reserves its legal rights. For this reason, the relevant form must be sent in a complete manner, including the requested information and documents, depending on the nature of your request.
6) RECOGNITION OF THE APPLICANT AND CONTACT INFORMATION ABOUT THE APPLICATION

In order to ensure the security of your personal data, ÖZPOR may contact you in order to confirm that you are the data owner, within seven (7) business days from the date your application for information is received by ÖZPOR, and may request some information and documents from you in this regard.
In case the requested information and documents are missing, the information and documents will have to be completed and forwarded to us upon our request. Until the information and documents are fully transmitted to us, the thirty (30) day period specified in article 13/2 of the Law regarding the conclusion of the request will be suspended.
